Quick answer: A gait belt is a sturdy 2-inch strap that goes snugly around a person’s waist so a caregiver has a safe handhold while helping them stand, walk or transfer. It does not replace a walker and is not for lifting. For most families, a 60-inch belt with a quick-release buckle, like the Posey E-Z Clean, is a good start.

Our picks at a glance

  • Best overall for home care: Posey E-Z Clean Gait Belt (60 in, wipe-clean, spring-loaded buckle).
  • Best with handles: SafetySure Professional Gait and Transfer Belt (5 handles).
  • Best for a larger waist: Posey E-Z Clean 81-inch (fits 51 to 76 in waists) or Medline Bariatric Gait Belt 70 in.
  • Best budget: OTC 2466 cotton gait belt with locking-teeth buckle.

What is a gait belt and who is it for?

A gait belt (also called a transfer belt) is a strong strap, usually 2 inches wide, that buckles around the waist over clothing. The person helping holds the belt, not the arm or the clothes. Pulling on an arm can hurt a shoulder or tear thin skin; a belt spreads the force across the waist.

It is most useful for families helping a parent who:

  • can bear weight and walk a little, often with a walker, but is unsteady;
  • needs a hand getting up from a chair, bed or toilet;
  • is walking again after a hospital stay or surgery, as instructed by a therapist.

Who a gait belt is NOT for

  • Someone who cannot bear weight. A gait belt is a handhold, not a lift. NIOSH research suggests a 35-lb maximum for manually lifting a patient in most tasks (Waters, 2007). If your parent needs to be lifted, ask about a mechanical lift or sit-to-stand device.
  • Use as a restraint or seat belt. Posey’s instructions say the belt is not a restraint and must never be used as a seat belt in a chair or car.
  • Recent abdominal or chest surgery, an ostomy, G-tube, hernia or lines at the waist – ask the care team first. Michigan Medicine’s guide says the belt can go higher, under the armpits, when there are abdominal wounds, but only as the clinician advises.

How do you use a gait belt safely?

These steps follow the Michigan Medicine falls prevention guide and the Posey instructions for use:

  1. Check the belt. Look for frayed stitches, tears or a cracked buckle. Do not use a damaged belt.
  2. Put it on at the waist, over clothes. Thread the end through the buckle and pull it snug. Tuck in the loose end.
  3. Check the fit. You should be able to slide a flat open hand between the belt and the body. Snug, not tight.
  4. Test the buckle. Pull on the belt to make sure it does not slip before every use.
  5. Grip from underneath. Stand slightly behind and to the side, and hold the belt from below with your palm up.
  6. Walk on the weaker side, close to your parent, while they use their walker or cane as usual.
  7. Take it off after use. Do not leave it on while sitting or in bed.

What should you do if they start to fall while wearing the belt?

Do not try to hold them up. Michigan Medicine’s guide says to pull the person toward your body and, if they cannot recover, lower them slowly to the floor along your leg while keeping hold of the belt. Then check for injury before helping them up. If they cannot get up easily, call for help rather than lifting.

Gait belt with handles or without?

Handles give a firmer grip, which many caregivers with weaker hands like. Plain belts are thinner, cheaper and easier to fit under a coat. Either works when used correctly. Choose handles if your hands tire, or if two people help with transfers.

What size gait belt do you need?

Measure around your parent’s waist over the clothes they usually wear. Pick a belt that fits that measurement in the middle of its range, not at the very end. A 60-inch belt fits most adults (Posey lists 28 to 55 inches). For a larger waist, choose a bariatric length like the 70-inch Medline or 81-inch Posey.

Does the research show gait belts prevent falls?

The evidence is thin. A 2019 review by Canada’s drug and health technology agency (CADTH) found no studies on the safety of transfer belts for partially mobile patients. Gait belts are widely used and recommended in hospital and therapy training, but they work best as one part of a plan: the right walker, a safer home and strength and balance work.

Can’t stand for the transfer? If your parent can sit up but cannot stand safely, a sliding board may be the better tool. See our guide to the best transfer boards for seniors.

Frequently Asked Questions

Can I use a gait belt at home without training?

You can buy one without a prescription, but ask a physical therapist or nurse to show you how to use it with your parent. A short demonstration makes it much safer.

Where should a gait belt be placed?

Around the waist, over clothing, snug enough that only a flat hand fits underneath. With abdominal wounds, a clinician may advise placing it higher.

Can you lift someone off the floor with a gait belt?

No. A gait belt is for guiding and steadying, not lifting. NIOSH research suggests a 35-lb limit for manual patient lifting. If someone cannot get up with light help, call for assistance.

Is a gait belt the same as a transfer belt?

Mostly yes. “Transfer belt” often means a wider belt with handles; “gait belt” usually means a plain 2-inch strap. Both are used the same way.

Does Medicare pay for a gait belt?

We could not find Medicare coverage for gait belts, so plan to pay out of pocket. Some retailers list them as FSA eligible.

How do you clean a gait belt?

Coated belts like the Posey E-Z Clean wipe clean after each use. Cotton belts can usually be washed; follow the maker’s label and let the belt dry fully before use.
