Quick answer: A swivel seat cushion is a padded disc or square that turns 360 degrees on a base. You sit sideways on the car seat, then the top turns as you swing your legs in, so there is less twisting of the hips and back. Most hold 250 to 350 lb and measure 15 to 18 inches across. Pick one rated above the user’s weight that fits the seat.

What does a swivel seat cushion do?
It works like a turntable. The user sits down sideways with feet still outside the car, then turns to face forward while lifting the legs in. The top of the cushion turns with the body, so the hips and lower back twist less.
Health guidance from HealthLink BC describes the swivel seat this way and also suggests other simple aids, such as grab handles and door straps (HealthLink BC, medically reviewed by a physician and a physical therapist).
The same cushion can help at a dining chair or desk chair. It does not lift you. If standing up is the hard part, see our stand-assist devices guide.
How do you get into a car safely with a walker?
These steps follow published hospital and health-service guidance (HealthLink BC; Northwestern Medicine, 2023; Frimley Health NHS, 2025). Ask a therapist to show you first.
- Park well. Choose level ground with room to open the door fully.
- Set the seat. Slide the front passenger seat all the way back. Recline it slightly if you need more room.
- Place the cushion. Put it flat on the seat. Check that it will not get in the way of the seat belt.
- Back up with the walker. Walk to the car, turn, and back up until the backs of your legs touch the seat edge.
- Hold something solid. Put one hand on the dashboard or door frame and the other on the seat back. Do not hold the car door – it can move.
- Sit sideways, then turn. Sit down slowly, watch your head on the door frame, scoot back, then swivel forward while you lift your legs in together. A helper can lift a sore leg.
- Buckle up. Fasten the seat belt. Fold the walker and store it.
To get out, reverse the steps. Have someone hand you the walker, and do not take it until you feel steady. A light walker that folds easily makes this much simpler.
Is a swivel cushion safe after hip replacement?
Many people use one, but follow your own surgeon’s precautions. Hospital leaflets for hip replacement tell patients to keep the operated leg straight, not to bend the hip past 90 degrees, and to avoid twisting the leg inward (Frimley Health NHS; Northwestern Medicine). Northwestern suggests an extra pillow on the seat so the hip does not bend past 90 degrees. A thin swivel cushion adds only about 1 to 2 inches.
For walker choices during recovery, see best walkers after hip surgery.
How do you choose a swivel seat cushion?
Weight limit
Pick a cushion rated well above the user’s weight. The models here are rated 250 to 350 lb. One listing does not state a limit at all.
Size and seat shape
A 15 to 16 inch round cushion suits flat seats. Deep bucket seats can tilt a round disc. A larger square cushion such as the 18 x 18 in Stander, or a flexible base, may sit better.
Thickness
Thicker padding is more comfortable but raises the user. In a low car, that can help you stand up. In a small car, check head room.
Grip and latex
A non-slip base stops the cushion sliding on leather seats. The Stander model contains natural rubber latex, so skip it if anyone has a latex allergy.
Who should NOT use a swivel seat cushion?
- Anyone who cannot sit up safely without support. A transfer board or professional help may be needed.
- People who cannot take their own weight to stand. The cushion only turns – it does not lift.
- Users heavier than the cushion’s rating.
- Anyone whose surgeon has given different car instructions after hip or knee surgery.
- The driver. We suggest using a turning cushion only in the passenger seat and removing it before anyone drives.

What else helps with getting out of the car?
A car handle gives you something solid to push on when you stand. The Stander HandyBar (item 3001) slips into the door latch striker and holds up to 350 lb. Stander says it works only with a U-shaped striker, not a striker bolt (Stander). We compare it with other aids in our stand-assist guide.
For a caregiver who helps a parent stand, a gait belt gives a safe handhold at the waist.
Does Medicare pay for a swivel seat cushion?
We found no Medicare policy that covers swivel seat cushions. Medicare covers durable medical equipment that serves a medical purpose and is used in the home (Medicare.gov). Its equipment reference list denies similar items, such as grab bars, as self-help devices “not primarily medical in nature” (CMS NCD 280.1). Most families pay out of pocket. Some Medicare Advantage plans offer over-the-counter allowances, so ask your plan.
Why safer transfers matter
About 1 in 4 adults aged 65 and older report a fall every year – over 14 million people (CDC). Getting in and out of seats is a common moment to lose balance. Not sure about a parent’s risk? Try our fall risk assessment guide.

Frequently asked questions
Do swivel seat cushions really work for seniors?
They help people who can sit and stand but find it hard to twist and lift their legs into a car. They do not lift you, so they will not help if standing up is the main problem.
How much weight can a swivel seat cushion hold?
The cushions in this guide are rated 250 to 350 lb. The Stander EZ Swivel Seat has the highest rating at 350 lb. Always check the listing.
Can I use a swivel cushion on a leather car seat?
Yes, but choose one with a non-slip base and check that it does not slide before you sit. Some round cushions tilt on deep bucket seats.
Can I use a swivel cushion after hip replacement?
Many people do, but follow your surgeon’s precautions, such as not bending the hip past 90 degrees and keeping the operated leg straight while you swing in.
Should the driver sit on a swivel cushion?
No. Use it in the passenger seat and remove it if you are driving, so the seat stays stable.
Does Medicare cover swivel seat cushions?
We found no Medicare policy that covers them. Medicare denies similar self-help items such as grab bars as not primarily medical. Ask your plan about any over-the-counter allowance.
